A safe discharge needs a provider who answers today. That is us.
Medium-term and SIL step-down, short stays, support workers and nurses for the first week home, and a coordinator who will sit in the family meeting. One number, answered 24/7.
- Same-day response Intake replies within minutes in business hours and the phone is answered around the clock, including weekends.
- Ward visits within days A coordinator meets the participant and family on the ward at Casey, Dandenong, Frankston or Latrobe Regional, or on video.
- Step-down options, honestly We tell you which of STA, MTA, SIL or home-with-support we can actually deliver, and when, before you plan around it.
What we can take off the discharge plan
Newdawn is a registered NDIS provider with its head office in Cranbourne, a short drive from Casey Hospital in Berwick and Dandenong Hospital, and within reach of Frankston and Latrobe Regional. We deliver hospital discharge support end to end: the accommodation the person steps down to, the workers and nurses who meet them there, and the coordination that keeps the NDIA, the family and the ward on the same page.
For participants with an existing plan we can usually start on existing funding while a variation is requested. For people who are not yet on the NDIS, we help the family and the ward social worker get the access request and evidence moving, and we are honest about what cannot start until it is approved.
Our specialty is the discharge others find hard: complex mental health, forensic involvement, behaviours of concern and high intensity health needs. Those referrals get a senior coordinator and a plan written for the first 72 hours, not just the first day.

How a discharge referral runs
- 01
Ward calls intake
03 5998 0717 or the online form, any day. Tell us the expected discharge date, the plan status and the clinical needs. We confirm receipt within minutes.
- 02
Senior coordinator on the ward
Within days, often the next business day for urgent cases. We meet the participant, family, treating team and the NDIA health liaison officer if one is involved.
- 03
Options and a 72-hour plan
We tell you which step-down options we can deliver and when, then write the first 72 hours: accommodation, roster, nursing, medications, equipment and who to call overnight.
- 04
Discharge, then the handover
Workers and nurses meet the person on day one. We report back to the ward within the first week and hand over to community services when the plan is stable.
What we need to say yes quickly
- Expected discharge date and whether it is fixed or flexible.
- NDIS status: existing plan (with budgets and how it is managed), access request in progress, or not yet started.
- Clinical needs that affect the roster: mobility and transfers, medications, wound or catheter care, feeding, seizures, mental state and risk.
- Any behaviour support plan, restrictive practice authorisation, supervision or treatment order, or safety plan.
- Who holds consent and decision-making, and the family's language and contact preferences.
- Equipment the person goes home with and anything still on order.
What you get back
- Confirmation and ND reference number the same day.
- A clear answer on each step-down option, with dates.
- A written 72-hour plan the ward can put in the discharge summary.
- A single Newdawn contact with a direct number, reachable overnight.
- A report back to the ward within the first week.
The hospitals we discharge from most
Casey Hospital in Berwick and Dandenong Hospital are closest to our Cranbourne office. We also work with Frankston (Peninsula University Hospital), Latrobe Regional in Traralgon, and the private hospitals along Kangan Drive. Our Caboolture and West Perth offices run the same model with their local hospitals.
Straight answers.
If yours is not here, call the intake desk. A person answers.
The person has a plan but no funding for accommodation. Can you still help?
Often, yes. Existing Core supports can usually fund workers at home from discharge, and a coordinator can request a plan variation for MTA, STA or SIL with the hospital's evidence. We tell you what can start now and what must wait for the NDIA, so the discharge plan is built on what is real.
How fast can a worker be at the bedside or the front door?
For an existing participant with funding in place, we have started supports within days of the first call, including same-week discharges. For a new participant, the limiting factor is the NDIA decision, not our roster. Urgent referrals go straight to a senior coordinator.
Do you take people from inpatient mental health units?
Yes. Psychosocial and complex mental health is a Newdawn specialty, and we work with the mental health units at Casey, Dandenong and Frankston. We write the discharge plan with the treating team, agree escalation lines for the first weeks, and our SIL for mental health page explains the model.
What about people with forensic involvement or orders?
We support people on community correction orders, parole, supervised treatment orders under the Disability Act and orders under the Crimes (Mental Impairment and Unfitness to be Tried) Act. Reporting obligations are agreed with the supervising authority before discharge. See our forensic disability accommodation page.
Who pays while the person is still in hospital?
The health system is responsible for care while someone is an inpatient. NDIS-funded supports start at discharge. Our ward visits and planning before that date are not charged to the participant's plan unless they already have support coordination funding and ask us to use it.
Can you train our patient's family or existing workers in the clinical tasks?
Yes. Our community nurses deliver the training and competency sign-off for high intensity tasks such as enteral feeding, catheter care and seizure management, for Newdawn workers and, where the plan allows, for other providers' workers and family members.
Call intake from the ward. A coordinator can be there this week.
03 5998 0717 is answered 24/7. Or send the discharge referral online and attach whatever you have; we will chase the rest.
Tell us what you need. We'll take it from there.
Whether you're exploring NDIS for the first time, looking for a support worker, or asking about one of our homes, we'll reply within a few minutes.
